How to Apply for PCSO Medical Assistance Program 2019

Let me share with you our story…

When my aunt was hospitalized and eventually had her one leg amputated, the PCSO had been a huge help to the family.

She was able to avail of the financial assistance from PCSO that helped her pay the hospital bills. PCSO also gave additional financial assistance when she bought her wheelchair and prosthetic leg.

We are all aware of how much hospitalization and medical expenses can cost now and we’re grateful that there’s PCSO who’s willing to help and serve people in need.

If you are currently suffering from an illness and your resources are not enough to pay your medication, you can actually ask for help from the PCSO through the PCSO Medical Assistance Program.

What is PCSO Medical Assistance Program or IMAP?

One of the programs that the Philippine Charity Sweepstakes Office (PCSO) offers is the Individual Medical Assistance Program or IMAP.

The main goal of the program is to provide financial assistance to all individuals with health-related problems through a guarantee letter.

A guarantee letter is issued to the hospital or partner health facility where the patient had availed of the medication or services.

This guarantee letter assumes the obligation of a specific amount due to the patient, but it’s not necessarily the full amount due.

The amount of assistance is based on the classification system established under the program.

Who can request for PCSO Medical Assistance?

All Filipinos with health-related needs may request for financial assistance from PCSO.

You may either apply personally or through a representative provided you qualify in the following criteria:

  • In-Patients or those confined in any health facility
  • Out-Patients or those who are receiving health care management outside the health facility
  • Filipino Patients seeking management in foreign countries (if no health facility within the Philippines is capable of providing such procedure

What Health Services are covered by IMAP and what are the Requirements?

The IMAP covers the cost of hospitalization confinement, medicines, chemo drugs, dialysis solutions, and other medical needs of patients.

To apply and avail of this program, you need to submit the general documentary requirements and the additional requirements specific to your needs which you can find below.

General Documentary Requirements

  1. Duly accomplished PCSO IMAP Application Form
  2. Original/Certified True Copy of the updated Clinical Abstract duly signed by the attending physician with license number
  3. Valid ID (Patient and Authorized Representative)
    • Government issued ID
    • Company ID
    • Student ID
  4. Authorization letter from the patient and Social Case Study in cases where there is no immediate relative available for interview

Specific Requirements

1. CONFINEMENT

Patients who are currently admitted or have been discharged with promissory note duly acknowledged by the health facility may avail of the PCSO medical assistance program.

Requirements: 

  • Hospital Bill or Statement of Account signed by the Billing Officer (original copy)
  • Endorsement Letter from the Medical Social Services of the Hospital or Health Facility for charity or service patients
  • PCSO IMAP Application Form – must be completely filled-out (you can get this from the hospital or at PCSO)
  • Updated Clinical Abstract – original or certified true copy and duly signed by the attending licensed physician
  • Valid ID of the patient and representative

2. DIALYSIS 

Patients who are undergoing dialysis such as Hemodialysis and Peritoneal (Continuous Ambulatory Peritoneal Dialysis or CAPD), may avail of the PCSO Medical Assistance program.

Requirements: 

  • Endorsement or Certificate of Acceptance of PCSO Guarantee Letter (If the hospital or health facility has a PCSO desk, this may not be required anymore)
  • Official quotation from service provider
  • Relevant laboratory results
  • Certification from Dialysis Center

3. CANCER TREATMENT – Chemotherapy and Radiation Therapy

Cancer patients who are undergoing therapies such as Chemotherapy and Radiation Therapy may apply for PCSO medical assistance program.

Requirements: 

  • Laboratory Results
  • Histopath / Biopsy Report (photocopy)
  • Prescription duly signed by the attending licensed physician (original copy)
  • Progress Notes from the Oncologist
  • Endorsement or Certificate of Acceptance of PCSO Guarantee Letter
  • Official quotation from service provider

4. MEDICINES

The PCSO also provides assistance for medicines such as Anti-rabies, Factor 8 and 9, Psychiatric, Post-transplant or post-operative, Antibiotics, and Intravenous Immunoglobulin (IVIG)

  • Laboratory Results
  • Histopath / Biopsy Report (photocopy)
  • Prescription duly signed by the attending licensed physician (original copy)
  • Progress Notes from the Oncologist

5. SURGICAL SUPPLIES

  • Official quotation with breakdown of expenses
  • List of operating room needs/supplies

6. IMPLANT (Bone/Cochlear)

  • Official sealed quotations from three (3) suppliers
  • Copy of the physician’s order with implant specification
  • Police Report (for medico-legal cases)
  • Date and schedule of operation
  • Copy of the result of relevant procedure i.e. X-ray, CT Scan and MRI

7. LABORATORY/DIAGNOSTIC PROCEDURES

  • Official Quotation from service provider with breakdown of expenses
  • Laboratory request duly signed by physician with license number

8. MEDICAL DEVICES (Pacemaker/Septal Occluder/PCI)

  • Official sealed quotations from three (3) suppliers
  • Copy of result of procedure, i.e. 2D echo, ECG and Angiogram

9. HEARING AID

  • Official sealed quotations from three (3) suppliers
  • Audiological Evaluation Report signed by a licensed Audiologist

10. WHEELCHAIR

  • Official sealed quotations from TahanangWalangHagdan
  • One (1) whole body picture

11. PROSTHESIS

  • Official sealed quotations from three (3) suppliers

12. PULMONARY APPARATUS

  • Official quotation from service provider

13. NON- & MINIMALLY INVASIVE PROCEDURES

  • Endorsement/Certificate of Acceptance of PCSO Guarantee Letter (GL)
    (Shall no longer be required in the case of facilities where the PCSO Desk has been established)
  • Relevant laboratory result
  • Costing of procedures

14. TRANSPLANT

  • Relevant laboratory result
  • Cross-matching result
  • Certification of patient’s inclusion in transplant program from authorized representative of National Kidney and Transplant Institute (NKTI), if applicable
  • Relevant Philhealth tracking number certification
  • Costing and breakdown of transplant expenses

15. REHABILITATIVE THERAPY (Occupational, Physical and Speech Therapy)

  • Official quotation from service provider with breakdown of expenses

Individual Medical Assistance Program (IMAP) Online Appointment

The online appointment system for IMAP Application is limited to securing a schedule for processing of documents or interview.

Preferred date of appointment is dependent on the existing manual appointment scheme of Charity Assistance Department.

Printed IMAP Application form is A MUST together with the original documentary requirements should be submitted on the date of appointment schedule at Radio-Onco Building, Lung Center of the Philippines at 8:00am to 3:00pm.

    Good day… Humingi po kami ng tulong sa PCSO last July 7 namin nagamit para sa CtSCAN.
    Ngayon kailangan pa po ng patient mag MRI. Itatanong lang po namin lung anu ano po ang kakailanganin namin? O kung maari na po ba kaming humingi ng tulong. Possible po bang ma grant?

    Pwede pa po ba kamiag apply kung nakagamit na c mama ko ng assistance sa bill nya last july 14, tapos for chemo po kac sya kac may cancer po sya nakita sa biopsy po. Pwede pa po ba kmi humingi ng assistance pra sa chemo nya sa july 27 po?

    pwede po ba kami humingi ng tulong wala po kasi kaming budget para sa dialysis ng papa ko, peritonial dialysis po sya! tsaka mga medicines po at injections. sobrang mahal po kasi ng capd solution bag nya parehas pong walang trabaho mga mgulang ko mula nung magkasakit si papa last june lang. salamat po!

  6. Hello, Alam ko po every 30 days or 45 days after ng assistance nila pwede magapply ulet. Cross check niyo po.. for the paid expenses parang hindi na po, dapat hindi pa paid lahat ng bills pra makapag grant sila. Thank you

  10. Bakit po kaya 11,000 lang ang na-grant para sa Tatay ko out of almost 400,000 bill. Nasa ICU po siya halos 2 weeks na ngayon. Ikaapat na confinement na po niya since May. Almost a month po ang 3rd confinement po nya.
